Whether you are a new client, have a new pet, or are dropping your pet off for the day, we will need for you to complete the appropriate form.  To help speed up your check-in, you may choose to print the form out and complete it before coming in [Adobe Acrobat required].  As you may have already experienced, filling out a form while holding your dog is often a struggle!  Please call us if you have any questions or leave any sections you cannot fill out blank and we will help you when you arrive.

If you choose to request a refill from this site, please give us 24 hours to have the prescription ready.  We will notify you via e-mail when the prescription is ready to pick-up.  If you do not receive an e-mail within 24 hours, please call us at 901/274-1444.
